Recently, the AADE’s monthly newsletters have centered on a prominent theme: the adoption of the Chronic Care Model as a guide for improving the quality of care for people with diabetes.
Over the next few months, the AADE's monthly newsletters (e-FYI) will focus on presenting a clearer, more in-depth view of the Chronic Care Model and what it means to professionals and patients who deal with chronic illness on a daily basis. It will examine the separate components of the Chronic Care Model, provide a glimpse of the Model in action, and provide resources and tips for implementing the Chronic Care Model in your practice.
